About the role:
The Dementia Together program, supported by CatholicCare, is a carer wellbeing and respite initiative designed to empower individuals living with dementia and their caregivers. We aim to enhance skills, provide support, and enable people to continue living at home for longer, more fulfilling lives.
As a Case Manager, you will play a pivotal role in delivering this program. You will conduct initial assessments to determine eligibility, provide guidance and referrals, and develop tailored care plans to meet the unique needs of carers and individuals living with dementia. You will also actively participate in program delivery in roles such as House Lead, Overnight Lead, or Engagement Assistant, ensuring smooth operations and an exceptional participant experience.
Key responsibilities of this role include:
🔹Conduct initial assessments to determine program eligibility and suitability.
🔹Provide personalised guidance, support, and referrals to address the specific needs and skill gaps of carers and participants.
🔹Develop individualised care plans to enhance participation in the Dementia Together program, incorporating education sessions where required.
🔹Participate in program delivery, assisting with set up, facilitation, and clean-up in line with documented processes.
🔹Act as a vital link connecting participants and carers with the resources and support they need to maximise their wellbeing and quality of life.
What we are looking for:
🔹Minimum 5 years’ experience supporting individuals living with dementia and their carers.
🔹Certificate of attainment in CHCAGE011 – Provide Support to People Living with Dementia (or willingness to obtain).
🔹Diploma of Community Services, Certificate IV in Community Services, or similar (highly desirable).
🔹Current C or CA class Queensland driver’s license.

🔹Dementia Knowledge & Awareness: Solid understanding of dementia, common symptoms, subtypes, and impacts on individuals and carers.
🔹Effective Communication & Collaboration: Ability to work with clients, colleagues, and stakeholders while managing information accurately; provide exceptional, person-centred service.
🔹Adaptability & Autonomy: Work independently, communicate openly, and demonstrate solution-focused thinking in a dynamic environment.
🔹Computer Proficiency & Data Security: Competent in Microsoft Office suite and able to adhere to data security and privacy standards; provide basic technical assistance or liaise with IT support when needed.
