About the Role
The Registered Nurse – Community Care is the senior nurse responsible for the care of clients in their own home as part of the Home Care Package Program and National Disability Insurance Scheme. This varied role includes a range of tasks including
🔹Provision of in-home nursing care
🔹Undertaking assessments to support clients accessing services and support
🔹Working collaboratively to develop and implement individual care plans for complex clients
🔹Supporting CatholicCare’s Clinical Governance Framework by providing expert advice to other staff regarding care and incident response for clients with complex needs.
🔹Proactively contribute to the development and implementation of policies and procedures to enhance the services provided to all clients
The Registered Nurse – Community Care will also provide leadership for the nursing service within the identified region/s and support and oversee other enrolled nurses providing in-home nursing care.
What We’re Looking For
🔹Bachelor of Nursing Degree
🔹Minimum 5 years of nursing experience
🔹Minimum 3 years community (or aged care) Nursing experience (Desirable)
🔹Current C class Driver’s license (QLD)
In addition to the above, you will need to demonstrate:
🔹Ability to assess client needs and develop appropriate plans for interventions and treatment for clients
🔹Ability to develop and provide reports regarding clients needs and conditions
🔹Ability to professionally work with internal and external stakeholders to meet client needs
