The Support Worker Community Care, provides high-quality, individualised care to clients, helping them to maintain independence and quality of life in their own homes. The role focuses on fostering social inclusion and participation, assisting with daily living activities and promoting autonomy while ensuring compliance with Aged Care standards.

Services are tailored to each client’s unique needs and preferences, aiming to meet their personal goals. Support includes:

  • Personal Care – assistance with daily personal activities, including bathing, dressing, grooming and hygiene;
  • Domestic Assistance – cleaning, laundry, meal preparation/cooking and maintaining a safe living environment;
  • Social Support – accompany clients to social activities, community / family / friend events, recreational outings and medical appointments;
  • Daily Living – support clients develop and maintain daily living skills, including shopping, budgeting, and cooking, whilst encouraging and guiding them to perform daily tasks independently where possible;
  • Medication – assistance with Medication (where suitably qualified);
  • Transport Assistance – provide transport for clients to attend medical appointments, therapy sessions or social engagements;
  • Respite Care – provide temporary care to give family or primary carers a break;
  • Additional Support – provide behavioral, emotional, mental health and technology assistance to assist clients manage their daily challenges effectively

  • Experience with personal care, medications, manual handling and behaviour support plans
  • Knowledge and skills gained through on-the-job training commensurate with the requirements of the work in this level

Core Competencies

  • Demonstrated ability to work independently in client homes following care plans and following a planned roster/schedule;
  • High level of customer service skills including the ability to support clients to build independence and exercise choice and control over their daily lives;
  • High level of demonstrated ability to provide services safely, with the ability to identify, assess and control risks as they occur within the workplace;
  • Demonstrated ability to perform manual handling tasks safely and effectively; and
  • Sound level of ability to use digital systems including email, messenger systems and client information systems to understand client needs, create case notes and other records and to record services delivered.

Qualifications and Experience

  • Current C or CA Driver’s license (QLD)
  • Certificate III or higher in a relevant qualification with a minimum of 12 months industry experience
  • Mandatory, current First Aid & CPR

In addition to the above, you will need to demonstrate:

  • Previous experience of working with culturally diverse clients
  • Experience in client focused roles
  • Ability to adapt communication to individual client needs
  • Ability to recognise risks and incidents, react appropriately and report accurately

The successful candidate will also be required to demonstrate entitlement to work in Australia, and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
