About the Role

As the Registered Nurse – Dementia Together, you’ll provide high-quality, person-centred care to clients in their own homes

In this varied and rewarding role, you will:

🔹 Deliver compassionate, in-home nursing care and clinical assessments

🔹 Develop and implement personalised care plans for clients with complex needs

🔹 Provide expert clinical advice and support to colleagues across CatholicCare

🔹 Contribute to clinical governance and continuous service improvement

What We’re Looking For

✅ Bachelor of Nursing and current AHPRA registration

✅ At least 5 years of nursing experience

✅ (Desirable) 3 years in community or aged care nursing

✅ Current driver’s licence (QLD)

In addition to the above, you will need to demonstrate:

💡 Assess and respond to complex client needs

💬 Communicate effectively with clients, carers, and other professionals

🧩 Develop reports and documentation to support quality care delivery
